What is Supported Decision-Making (SDM)?
Supported Decision-Making (SDM) is a collaborative approach that empowers individuals with disabilities to retain control over their lives while receiving guidance from trusted supporters. Instead of guardianship, which removes decision-making rights, SDM ensures individuals stay at the center of choices about healthcare, education, finances, and more.

How can PRAGNYA Help!
Recognizing the importance of self-determination, PRAGNYA's SDM TAP team will provide crucial SDM Navigation services to individuals with developmental disabilities and their families. Our program will offer personalized guidance through the process of understanding, establishing, and practicing Supported Decision-Making. This includes one-on-one facilitation, supporter training, and the development of SDM Agreements. With the support of the SCDD, PRAGNYA is committed to expanding access to SDM and ensuring individuals have the tools and support they need to lead self-directed lives.
